In the middle of a benzodiazepine taper, the numbers get small. Smaller than tablets. Smaller than half tablets. Smaller than anything a pill splitter could dream of producing.
This is not a problem. It is the plan working. Hyperbolic tapering means the reductions shrink as the dose shrinks, because the relationship between dose and effect is not a straight line. Near the end of a taper, a change of a fraction of a milligram can matter as much as a whole milligram mattered at the start.
The question is how anyone measures a step that small. One answer, elegant in its simplicity, is the two-syringe method.
Think of a kitchen, not a laboratory
No one measures a quarter teaspoon of vanilla with a coffee mug. And no one measures four cups of flour with a quarter-teaspoon spoon, scoop by maddening scoop. A kitchen solves this with two tools. A big measure for the bulk. A small measure for the fine amount. Together they hit the recipe exactly.
The two-syringe method is that same wisdom applied to a liquid benzodiazepine taper. A compounding pharmacy prepares the medication as a liquid, and it can prepare that liquid at more than one strength. One bottle is stronger, meaning more medication in each milliliter. One bottle is weaker, meaning much less medication in each milliliter.
The stronger liquid is the measuring cup. It carries the bulk of the dose in a small, manageable volume. The weaker liquid is the quarter-teaspoon spoon. Because it is so dilute, a visible, easily measured amount of it contains only a sliver of medication. That sliver is where the precision lives.
One clean illustration
Here is the idea with invented numbers, shown only to make the concept visible. Imagine a dose of 5.2 milligrams. A stronger liquid might supply the first 5 milligrams in one syringe. A much weaker liquid might supply the remaining 0.2 milligrams in the other. When the next reduction arrives, the first syringe does not change at all. Only the small syringe draws a little less.

That is the beauty of it. The big number holds still while the small number glides downward in steps a tablet could never take. Real concentrations, volumes, and schedules are worked out by the physician and the compounding pharmacy for each individual patient, which is why this article shows the shape of the method and not a recipe. A taper is medical care, and this technique only works properly with medical oversight.
Why bother with this much precision?
Because the nervous system of a person tapering a benzodiazepine is exquisitely sensitive to change. Physical dependence is not addiction. It is an adaptation, and undoing an adaptation gently means changing as little as possible at a time. This is the thinking behind micro-tapering, where reductions are very small and sometimes very frequent, and it is consistent with the hyperbolic approach described in the Maudsley Deprescribing Guidelines and the gradual, patient-paced philosophy of the Ashton Manual.
Pills cannot follow that curve to the floor. Even liquid from a single bottle has limits. If the liquid is strong, the final reductions become droplets too small to measure with confidence. If the liquid is weak enough to measure those tiny steps, the full dose at the start of the day would fill a volume nobody wants to swallow. Two concentrations solve both problems at once. Each syringe stays in the comfortable middle of its range, where measurement is easy and repeatable.
What this asks of you, and what it does not
Here is the reassuring part. The method sounds mathematical, but the math is not your job. Dr. Leeds, who builds liquid tapers with compounding pharmacies as a central part of his tapering practice, treats the calculations as the physician’s work and the preparation as the pharmacy’s work. The patient’s contribution is consistency. Same liquids, same syringes, same routine, every day.
Consistency is also why the method comforts so many people who use it. There is no eyeballing, no wondering whether today’s dose matched yesterday’s. The plunger stops at a printed line, and the line does not move unless the plan says so.
A taper can feel like standing on a long staircase in the dark, unsure how tall the next step down will be. Precision dosing turns on the light. The steps become small, known, and evenly spaced, and the nervous system, which loves nothing more than predictability, gets to descend at a pace it can absorb.
